Old Students

1. Accomplished Enrollment Forms
2. Original Copy of the Birth Certificate (if unable to pass upon the first year of enrollment)
3. 4 pcs. 1X1 picture of the child
4. 4 pcs. 1X1 picture of the regular fetcher

New Students

1. Entrance Exam and Interview
2. Accomplished Enrollment Forms
3. Original Copy of the Birth Certificate
4. Letter of Recommendation from the previous school
5. Original report card / Scholastic records of previous school attended
6. 4 pcs. 1X1 picture of child
7. 4 pcs. 1X1 picture of the regular fetcher

For Filipino Citizens

1. Two photo copies of birth certificate (bring original copy for verification)
2. One photo copy of Health record of the child (include date of immunization)
3. ID or photo copy of Certificate of Naturalization for naturalized Filipino

For Aliens

1. Two photo copies of birth certificate (bring original copy for verification)
2. One photo copy of Health record of the child (include date of immunization)
3. Two photo copies of ACR (bring original copy for verification)
4. Study permit (original copy) for non-permanent resident
